The dossier is scoped to the case. Oncology fields may include stage, pathology and biomarkers; non-oncology fields may include specialty-specific tests, imaging, procedure and anaesthetic history, implant or device make and model, current programming, rehabilitation, medicines, organ function, infection risk, and the exact clinical question.
The hospital response should state the product, procedure, device or protocol; exact site and responsible specialty team; approved, off-label or research status where relevant; alternatives; principal risks; schedule; ICU and rescue pathway; and required follow-up.
Materials can include the itemised hospital estimate, procedure and device codes, implant specifications, clinical rationale, anticipated admission and rehabilitation, complication and revision terms, network details, direct-settlement route, and patient-liability acknowledgement.
The return packet should contain treatment delivered, operative or procedure report, pathology, dosimetry or device record as applicable, complications, medicines, restrictions, urgent warning signs, wound or programming plan, rehabilitation and monitoring schedule, responsible contacts, and specialty-specific home follow-up.
The initial public enquiry remains minimal; professional records use a separate secure process.
